A small headline increase with big market impact
The Centers for Medicare & Medicaid Services (CMS) released its 2027 Medicare Advantage and Part D Advance Notice, projecting a net average year-over-year payment increase of 0.09% for Medicare Advantage plans in 2027—roughly $700 million in additional payments. The proposal landed as a surprise to many market watchers who had expected a larger lift, given persistent medical-cost pressures.

Shares of major insurers fell after the release as investors digested what a near-flat update could mean for profitability. Companies that rely heavily on Medicare Advantage membership face the risk that premium and benefit designs for 2027 may need to tighten if costs continue rising faster than reimbursement.
Why Medicare Advantage rates matter
Medicare Advantage is the privately run alternative to traditional Medicare, funded largely by federal payments. When the payment update is small, insurers have less room to absorb higher utilization and unit costs. Analysts warned the proposed rates could force benefit reductions, premium adjustments, or even plan exits in some markets if margins become unsustainable.
CMS emphasized that the Advance Notice includes technical updates meant to improve payment accuracy and sustainability. The agency also noted that additional elements, such as risk score trends driven by coding practices and population changes, can alter the effective payment change experienced by plans beyond the 0.09% net figure.
What could change before 2027
The Advance Notice is a proposal, not a final rate announcement. CMS typically takes public feedback and may revise elements before finalizing. Industry groups and provider organizations quickly signaled concern, arguing that instability or underpayment can ripple to patients through narrower networks or less generous supplemental benefits.
Over the coming months, insurers will model how the proposal affects county-level benchmarks, star-related bonuses, and risk adjustment, then decide how to price and structure 2027 plans. For beneficiaries, the practical question is whether 2027 brings higher out-of-pocket costs or scaled-back extras like dental, vision, or supplemental benefits that many Medicare Advantage plans have used to compete.
